People in pain can often become sensitive to gentle touch or simple movements. Sometimes this sensitivity can spread far away from the originally painful and/or injured area. This feature has been referred to as ‘central sensitisation’ by a number of prominent researchers. But, what is central sensitisation and is this change in sensitivity a true representation of it? We will explore the basic sciences of central sensitisation and ask if it offers us clinically relevant knowledge.

Plasticity is an important biological feature of learning and memory formation. It has been suggested that plasticity changes are present in pain and that some of these changes underpin the transition of acute to persistent pain states. Is there really a dark side to plasticity? If so, what is it?

Ron Melzack proposed the Neuromatrix model to better understand complex pain states such as phantom limb pain, complex regional pain syndrome and pain following deafferentation injuries. Influenced by this model, researchers and clinicians have devoted more attention to brain processes assumed to be involved in a pain experience. Treatment approaches like Graded Motor Imagery have been developed and appear to offer hope for these complex pain states. Does that mean we can target the brain with specific treatments? Is retraining the brain reality or fantasy?
